Coherent patient pathways for recent diagnosed patients / citizens having COPD

Short summary

The purpose of the project is to ensure that the Hospital of Southern Jutland to a greater extent refers people with COPD  to preventive treatment in the municipality, and that the patients / citizens accept the offer. This also includes ensuring that people with COPD get the right information about the prevention offers, and also that the municipality gets the necessary information from the hospital.


Rationale

Danish national studies show that some COPD patients / citizens do not participate in rehabilitation programs in the municipality. This is despite the fact that there is evidence that early rehabilitation prevents re-admissions.

The purpose of the study is to ensure that more COPD patients / citizens participate in rehabilitation programs in the municipality.


Description of the cohort

Recently diagnosed COPD patients living in the municipality of Sonderborg.


Data and biological material

Patient questionnaires will be collected. Futhermore data on referral and community uptake will be collected.
