Vulnerability & Palliative Care at the Hospital

Short summary

We aim to improve the palliative care experience for socially vulnerable patients at Odense University Hospital (OUH) by optimizing access, coordination, and quality of care. Data collection methods: focus group interview, questionnaire, PROMs. The study explore: 1. competencies managing palliative care pathways for vulnerable patients 2. the reliability of screening patients using the Surprise Question, SPICT, and speciality-specific criteria as to identify patients in need of palliative care.


Rationale

Socially vulnerable patients, such as those experiencing homelessness, mental illness, substance abuse, or social isolation, often face significant barriers to accessing palliative care. Socially vulnerable patients frequently encounter systemic inequities that hinder their access to palliative care. These patients are at a higher risk of inadequate symptom management, delayed referrals, and fragmented care pathways. The provision of palliative care to socially vulnerable populations is a critical area of concern within healthcare systems. Barriers to effective palliative care delivery encompass both organizational and individual factors. With this project, we aim to improve the palliative care experience for socially vulnerable patients at Odense University Hospital (OUH) by optimizing access, coordination, and quality of care. Research questions 1. What are the barriers preventing socially vulnerable patients from accessing timely and adequate palliative care? 2. What strategies could improve care coordination in socially vulnerable patients in a palliative care context? 3. What is the impact of these interventions on patient outcomes and healthcare provider experiences?


Description of the cohort

Participants are multidisciplinary healthcare professionals. The study will be conducted in the geriatrics department and departments of oncology and cardiology at OUH.


Data and biological material

Questionnaires from healthcare professionals and patients; statements from focus group and individual interviews. Data retrieved from electronic health records: demographic, diagnosis, treatment, symptomburden.
