Temporary care homes provide short-term care and help alleviate hospital pressure. However, patient transitions increase the risk of medication errors due to changes and poor communication. This study will use an exploratory sequential mixed methods design to investigate and evaluate the transfer of medical responsibility during patient transitions from hospital to temporary care.
Our healthcare system is facing significant and sustained pressure due to an aging population and a growing demand for healthcare professionals, a trend expected to continue in the foreseeable future. Temporary care homes outside hospitals play a vital role in providing short-term care and rehabilitation, helping to alleviate pressure on hospital resources. However, patient transitions between hospitals and temporary care increase the risk of medication-related problems, often caused by medication changes during hospital stays and inadequate communication between sectors. Understanding the factors that ensure a safe and effective handover of medication responsibility is crucial to preventing medication errors. This study aims to investigate and evaluate the transfer of medical responsibility during patient transitions from hospital to temporary care, with a focus on healthcare professionals' acceptability, communication, and relationships involved in this task-shifting process.
