Derived effects of organizational changes in the health-care system on rehabilitation, health-care contacts and home-care services.
The study aims to use cross-sectorial data (from OUH, FAM and Odense municipality to investigate the derived effects of organizational changes in the health-care system on rehabilitation, health-care contacts and home-care services. Our hypothesis is that changes in one part of the healthcare system cause changes in other parts of the healthcare system which changes referral patterns and have health economic and treament consequenses.
The establishment of FAM means that the diagnosis and treatment of acute patients is moved from specialized departments to the FAM. by establishing FAM a larger proportion of patients is treated at FAM without admission / transfer to a hospital ward. This means for some patients a shorter initial course and earlier discharge. The concern is that patients might not at the same habituation level at discharge as before hospitalization. This may mean altering the level of home care or alternatively the patient should be discharged to another care service. An accelerated hospitalization process might affect communication between FAM and the municipality, so changes in care or municipal rehabilitation are missed in the discharge process, with the consequence of poorer functional level, quality of life, and perhaps re-hospitalization.
Description of the cohort
All patients from 2007 - 2017 dicharges with COPD, stroke, heart faliure and hip fracture, supplemented with data from Odense Municipality from 2015 - 2019 on the same patient categories.

We collect registry data and data from medical records .
