The diagnostic value of Dermal-Optical Coherence Tomography (D-OCT) for clinically suspected Basal Cell Carcinomas lesions (BCC) in the periocular area

Short summary

The primary objective of this prospective clinical study is to assess the diagnostic accuracy (sensitivity and specificity) of advanced scanning modalities, D-OCT (VivoSight) in identifying BCC lesions within the periocular region. These findings will be compared with earlier reports on D-OCT diagnostics for BCC lesions located outside the periocular region. Furthermore, this study investigates the diagnostic accuracy in subtyping and delineation of periocular BCC tumor extensions using D-OCT.


Rationale

D-OCT efficacy in diagnosing BCC outside the periocular area, is well established, with reported sensitivity and specificity of 95% and 77% respectively. D-OCT may also be used to determine lesion depth and extension prior to surgical excision. By utilizing D-OCT for margin delineation, it becomes possible to minimize resection margins without compromising complete removal. A smaller study demonstrated that D-OCT accurately defined lateral margins in 84% of cases. Presently, only two case-series studies have investigated the utility of D-OCT in diagnosing periocular BCC, encompassing 15 cases of nodular BCC and 46 cases, respectively. Accordingly, in order to reveal the potential of D-OCT in diagnosing BCC and lesions suspected to be BCC in the periocular region, assessment of the method sensitivity and specificity is needed.


Description of the cohort

Patients older than 18 years with clinically suspected periocular BCC.


Data and biological material

Age, sex, clinical observation using slit lamp (e.g. tumor location, size and appearance), digital photograps and D-OCT scans of the lesion before punch biopsy and pre-operativ, and a review of histopathological diagnoses and subtypes from both punch biopsy specimens and tumor samples.
