The AMETO Database - A Quality Assessment
Short summary
The AMETO Database concerns treatment of young patients with congenital missing teeth ( dental agenesis ) , mineralization disturbances, ectopic tooth development, trauma, and other diseases and was jointly developed by specialists within the field of orthodontics, pediatric dentistry, oral & maxillofacial surgery and general dentistry and accepted , 2009, as a clinical quality register by the Danish Health Care System.
Rationale
The AMETO register is founded as a quality assurance database, established by professionals with an odontological background and has been developed with the introduction of The County Dental Care in 2001 - later The Regional Dental Care in 2006. The Regional Dental Care is an offer for primarily children and young persons who due to congenital missing teeth, dental agenesis, have a need for treatment that left untreated will cause lasting loss of function. The Regional Dental Care is a specialized dental offer established in the Health Care Law §162, 163, and 166,3 and described in The Danish Health Care Authority's instructions. The main conditions are lack of permanent teeth, lack of enamel and tooth loss due to accidents. In conjunction with the initiation of the Regional Dental Care ,the Danish Health Care Authority §162, described a need of collection of knowledge regarding implant treatment of the patients with dental aplasia. It concerns young patients who preferably are offered a life long treatment, however no systematic knowledge has previously been collected concerning the quality of this care.The Regional Dental Care is teambased comprising the fields of orthodontics, prosthodontics, oral surgery and pedodontics. This requires a close cooperation between the Regional Dental Care and The Municipal Dental Service, since most of the pre-dental implant treatment such as orthodontics is supposed to be carried out in the latter. The patient is referred to the Regional Dental Care once a diagnosed dental agenesis requires treatment.
From 2001 until 2006, a quality assurance register, AMETO, was developed, with Professor Else Marie Pinholt as the initiator and project manager. Professor Pinholt is also the main supervisor in the current Ph.D study and the AMETO Register was acknowledged by the Danish Health Care Authority in 2009.
The AMETO register includes indicators that relate the actual treatments to those acknowledged in the international literature regarding patients with dental implants. However, no records exist solely with the focus on treatment of dental aplasia as a whole.
Treatment data has been registered in the AMETO register in the Region of Southern Denmark during the years 2007-2012 and for follow-up until 2016. Thereby, 4-5 years prospective follow-up exists where quality assurance on the outcomes and treatment results can be evaluated on dental replacements on patients who have been born without permanent teeth. This is the purpose of the Ph.D Project "AMETO- A quality Database". Data from the AMETO register from this period form the foundation for the Ph.D student's Vlad Florescu's Ph.D. project The data from the AMETO register comprise clinical data including x-ray data and patient questionnairies. At least three-four publications during the current Ph.D study are planned.
The studies include evaluation of treatment outcomes including patient questionnairies and related to the indicators of the register, and evaluation ot the register itself by the level of details, compliance, missing values etc.
Description of the cohort
The Cohort concerns 227 young outgrown patients with a diagnosis of dental agenesis and treated with dental implants in the Region of Southern Denmark from year 2006-2012 .
Follow-up was carried out at 0, 1, 3 and 5 years postoperatively.
Follow-up was between 2010-2017. End of Data collection is 31.12.2020 (evaluation and measurements of acquired x-rays during diagnosis, treatment and the follow-up period).
Data and biological material
Register data is collected, as well as questionnaires (Oral Health Impact Profile) and x-rays are acquired during the mentioned period, Measurments from x-rays remain to be determined in order to obtain data concerning bone level changes around the dental implants, which is an important outcome measure. Clinical data, including survival of implants, esthetic outcome evaluated by patient and the health care professional.
