Data-driven Quality Development of Acute Team Odense

Short summary

The aim of project Data-driven Quality Development of Acute Team Odense is to provide new systematic knowledge about the Acute Team Odense and the cross-sectoral collaboration. The project includes cross-sectoral data that makes it possible to examine the entire patient care in the acute healthcare system. 


Rationale

The Danish Acute healthcare system is under development due to the demographic development and it requires new directions in delivery of acute healthcare services. Strengthening the primary care with highly qualified nurses is one new direction. On January 1st, 2018, the city of Odense, Denmark, started the Acute Team Odense with the purpose of delivering acute nursing in the citizens home 24/7 through a collaboration with general practitioners, the emergency department, the geriatric department, other health professionals in the city of Odense, the out-of-hours service, the medical helpline and the ambulances. Knowledge about strengthening the primary care with highly qualified nurses is currently limited why new systematic research is needed. 


Description of the cohort

The cohort include patients who have received nursing interventions by the Acute Team Odense. The target group for the Acute Team is patients above 18 years.
